This position will be responsible for coordination and implementation of the Patient Centered Medical Home (PCMH) model in multiple Primary Care offices.
The Coordinator will lead Primary Care offices in developing tools for population management, chronic disease management and transitional care coordination. These tools will include data collection on clinical office flow metrics, pre-visit planning, post hospitalization planning, group visits, as well as disease management protocols. Our lead Primary Care office is currently in the process of attesting for NCQA PCMH level 3 statuses. This position fulfills the need for a coordinator to assist with implementation of this model in our other primary care offices. The Coordinator will work directly with the Medical Directors, Office Managers and Office Physicians for PCMH implementation. The ideal candidate we are looking for should have experience in both inpatient and outpatient care; strong communication skills; appreciation for quality improvement processes and change management. Must be able to inspire teams for creating an environment of passionate, quality healthcare delivery with a focus on continual improvement.
