The Clinical Advocate’s primary responsibility is to assist with population management and development of advanced medical home model in primary care offices. The advocate will communicate policies and protocols and workflow changes to primary care offices. The advocate will assist with implementation of changes related to population management and ensure compliance with policy and procedure. This includes monitoring population management reports, pre-visit planning, and consistent office communication in the form of huddles and monthly office meetings. As part of advanced medical home model the Clinical Advocate will assist in health coaching and support implementation of patient of care plans. The advocate will work under the direction of a supervising care manager/physician, and serve as a liaison between a primary care office and the Care Management Team/administration.
