Clinical Advocate - CMA/RMA or LPN - Full Time

Clinical Advocate is responsible for patient care coordination in the advanced medical home model for primary care offices. The Advocate will assist physician offices in implementing office policies and procedures for improved patient care while working under the direction of a clinical care supervisor. 

Duties And Responsibilities:

  • Work as a liaison between the Care Management team and a primary care office to ensure adoption and compliance with the Advanced Medical Home model.
  • Receive training and attend meetings with other clinical advocates to ensure uniform delivery of office policy and protocol.
  • Ensure that “best practice” policy and protocols are shared with office clinical staff and physicians.
  • Provide assistance with office workflow changes and use of protocols.
  • Ensure compliance with workflow changes and use of protocols.
  • Ensure compliance with documentation in appropriate EMR fields.
  • Assist and provide oversight for population management reports, including outstanding lab, radiology and referral reports; preventive services; recall processes, and chronic disease registries, under the direction of the supervising care coordinator.
  • Assist and provide oversight in primary care offices in the adoption of population management tools, including organization of “best practices” educational tools and community resources for patient care.
  • Share and discuss quality measures with office staff and providers.
  • Receive training and assist in development of quality improvement plans

Knowledge And Skills Required:

  • Capable of working with diverse social, racial and ethnic groups and collaborate with professionals from many different social service organizations and government agencies.
  • Ability to prioritize and plan work activities, use time efficiently and develop realistic action plans.
  • Comfortable with use of computers, capable of effectively utilizing word and excel programs.
  • Ability to identify and resolve problems in a timely manner and gather as well as analyze information skillfully.
  • Ability to adapt to changes in the work environment, manage competing demands and ability to deal with frequent change, delays or unexpected events.
  • Demonstrates dependability by being consistently at work and on time, follows instructions, responds to management direction and solicits feedback to improve performance.

EDUCATION AND WORK EXPERIENCE:

  • Completion of an accredited Licensed Practical Nursing (LPN) program or Medical Assistant (Registered or Certified) program
  • Minimum of two years’ experience in health care setting preferred.

CERTIFICATION/LICENSES:

  • Unrestricted, Licensed Practical Nurse by the State of Ohio or Certified/Registered Medical Assistant

ENVIRONMENTAL/WORKING CONDITIONS:

  • Combination of office and exam offices. Frequent exposure to communicable diseases, bodily fluids, toxic substances, medicinal preparations. Hepatitis B vaccination and TB test required.

PHYSICAL/MENTAL DEMANDS:

  • Varied activities including sitting, walking, stooping, lifting, bending, reaching. Occasional stress from workload.
